Patient e-Assessment Portal

Please note – Anyone can request an e-Assessment, you do not need to be a current or past patient of our clinic.
We only accept patient requests for e-Assessment from patients with a valid Ontario Health Card.

Our e-Assessment Team process requests seven days a week between the hours of 8am and 5pm.

You will need to upload pictures of both eyes to complete the form – see instructions HERE. 
You will also need to enter results of an online visual acuity test HERE.
